A new collaboration aims to narrow down the vast number of potential drug substances that could be studied for Kleefstra syndrome, using RNA from individuals with the diagnosis and computational models. IDefine and Unravel Biosciences announced on April 7, 2026, that they plan to collect non-invasive nasal samples from 20 participants and screen for substances that may influence molecular changes associated with the syndrome.<\/p>\n\n\n\n

The project is not a drug clinical trial. No drug was announced to be administered to participants, and there is no randomized clinical study within the framework of this cooperation. The human component is observational collection of molecular data<\/strong> from RNA samples. The search for candidate substances will be conducted computationally and belongs to a very early, preclinical stage of discovery.<\/p>\n\n\n\n

From the nasal sample to a computational model<\/h2>\n\n\n\n

According to the announcement, participants will provide a nasal swab sample, from which RNA will be analyzed. The RNA profile, or transcriptome, is a snapshot of the genetic instructions cells are using. Measuring it does not change a person's DNA and is not gene therapy. Because the collection was designed to be done remotely, it may reduce participation barriers for families living far from specialized research centers.<\/p>\n\n\n\n

Unravel plans to use the rareSHIFT and BioNAV platforms to create individual digital models of the molecular disease profile. The term \u00abdigital patient model\u00bb does not describe a literal copy of the human. It is a computational representation based on available biological data and is used to formulate research predictions.<\/p>\n\n\n\n

For each participant, the program provides for the computational screening of more than 40,000 molecules. The goal is to identify substances that, according to the model, could shift gene expression patterns associated with the disease toward a healthier state. The numbers of the 20 participants and the more than 40,000 molecules per person are parameters of the planned program<\/strong>, no results.<\/p>\n\n\n\n

Why drug repositioning is being considered<\/h2>\n\n\n\n

EHMT1 deficiency affects the regulation of many genes and may cause widespread changes in the way cells use genetic information. The rationale of the collaboration is to measure some of these changes and look for substances that might compensate for related processes. Unlike gene replacement, this strategy does not attempt to directly restore EHMT1 itself, but rather to influence downstream consequences in the biological chain.<\/p>\n\n\n\n

Drug repositioning or repurposing means examining whether a drug developed or approved for another condition could be useful in a new situation. The existence of prior information about a drug can facilitate certain stages of research. However, it does not mean that the drug is automatically safe, suitable, correctly dosed, or effective for people with Kleefstra syndrome.<\/p>\n\n\n\n

So far, no prioritized drug candidate, biomarker, cohort of potential responders, laboratory confirmation, or clinical outcome has been announced by this program. There are no observational data showing that any drug has benefited people with Kleefstra syndrome in the context of the collaboration. Furthermore, there is no randomized clinical trial that has tested the efficacy or safety of a candidate drug for this specific project.<\/p>\n\n\n\n

What can predictions prove<\/h3>\n\n\n\n

A computational prediction is a hypothesis to be tested, not proof of a therapeutic effect. The fact that a molecule appears to reverse an RNA pattern within a model does not prove that it will reach the appropriate tissue, that it will have the desired action in the brain, or that it will be safe. Computational tools can help with prioritization, but they do not replace laboratory testing, medical supervision, and clinical trials.<\/p>\n\n\n\n

There is also the issue of tissue. Cells from the nose are accessible and can provide useful information, but they are not neurons and do not capture the entirety of brain biology. It must be proven that the molecular signals identified are stable, relevant to the syndrome, and appropriate for the selection of candidate substances. An exploratory cohort of 20 individuals may be reasonable for an early-stage rare disease, but it cannot establish clinical efficacy.<\/p>\n\n\n\n

If interesting candidate substances emerge, confirmation will be needed in appropriate laboratory systems, possibly in cells derived from patients and in suitable animal models. Only after sufficient preclinical data could a properly designed human study be considered. Even for an already approved drug, an evaluation specific to the population, the target, the dosage, and potential adverse events would be required.<\/p>\n<\/div><\/div>\n\n\n\n

What it means for families today<\/h3>\n\n\n\n

The immediate significance of the program concerns primarily participation in research and the effort to systematically prioritize candidate drugs. The remote collection of nasal samples can allow the contribution of families who would otherwise find it difficult to travel. If the project achieves its early goal, it could narrow a huge search field down to a smaller number of candidates worth testing more rigorously.<\/p>\n<\/div><\/div>\n\n\n\n
